Integrated Self Care in Family Practice

 

 

The College Surgery at the Culm Valley Integrated Centre for Health is a large family practice in new premises in Cullompton, near Exeter in Devon, England. SustainCare has been working at the Centre to develop a range of guides in the use of simple home remedies, and will be working with the Culm Valley Patient Group to engage the interests of those with most interest in learning how to manage their health at home.

The pilot project includes a new physic garden at the entrance to the practice, and future exhibits and activities, including those for children and also a functioning vegetable garden in the car park. At the core of all the community activities is Café Sustain a vibrant community hub.

 

In January 2009 we embarked on an exciting new 2-year fully-funded project Integrated Self Care in Family Practice, to develop better ways to support patients in looking after themselves using simple home remedies and other accessible approaches. We are working with the staff and patients at Culm Valley, with two universities and Bromley by Bow a leading NHS practice in London. This work is based in the communities of patients and with their help we will have innovative new ways to help people look after themselves and their families.
